I do all of these things but I’m still confused on diet. I eat low saturated fat diet with oats, beans and sweet potatoes. I only eat sardines and salmon for my protein. Should I ditch the carbs and eat mono and polyunsaturated fat ketogenic diet?

Gerald, have you used a CGM? For me, oats and beans would definitely spike my glucose. I do steel cut overnight oats occasionally, and I eat them after a fat containing breakfast (eggs/avocado), but glucose will still go to 150. I also eat sardines and salmon, as well as mackerel. I hope to have some lists to upload in the near future with 2 weeks of APOE4 compliant menu options to rotate. Whether or not to ditch the carbs depends on how insulin sensitive you are.

I am very insulin sensitive, I run/walk everyday for at least an hour, I also lift weights 5 days a week, I also use a GLP-1. I have used a CGM my carb meals wont increase my blood sugar above 140 but it drops fairly quickly. My fasting insulin is below 3, my fasting glucose is below 85 with a Homa IR below 1. I still get conflicting thoughts on what the ideal diet that APOE4’s should eat. Blood sugar and heart disease both are our Achilles heel, I keep my APOB below 75 also. With my overnight oats, they do have flax seeds, walnuts and berries. When I eat beans they are prepared with olive oil and vegetables. Thank you for your reply, you are providing great resource for prevention. I also use red light therapy daily, 40hz light, rapamycin weekly and B vitamins to keep my homocysteine below 8.

sounds like you are in a great place! lf my blood sugar stayed below 140, I'd be eating more beans as well. Loaded with fiber (that's another biggie for us!) and healthy! We are so lucky to know our status and be able to do something about it for a course correction!
